To request verification of enrollment, please provide the following information in writing:

 ·Your full name at the time of enrollment
 ·User ID (student ID number or social security number)
 ·Major and degree sought

Also state which of the following should be included:

 ·current enrollment only
 ·enrollment for each term attended
 ·anticipated date of graduation

Verification of enrollment will be mailed within five (5) business days from the date the request is received. Mail or drop-off your request to:
