New Employee Paperwork

In order to expedite the new hire process, please complete the following forms and bring them with you on the day of your appointment. 

All New Hires are required bring original documents as we can not accept photocopies. Additionally, you will need to show us your social security card as proof of your identity.

 

1. Employment Application:
All new employees are required to complete this form.
1A. Faculty Application: Only to be completed by New Full-Time Tenure Track and Part-Time Adjunct Instructors.                                           
2. Background Check Consent and Disclosure:
As part of the procedure for processing the application of employment all applicants must fill out a background check consent and disclosure form. Please print out and complete this form.
2A. Article 23-A NY Corrections Law:             The State of New York requires organizations that obtain a consumer report from employment applicants provide the applicants with a copy of Article 23-A of the NY Corrections Law.  Please acknowledge receipt of this Article within the Background Check Consent and Disclosure form (#2) above.
3. Employee Acknowledgement:
This document confirms you have received the college's Employee Policy Manual-please follow the link to view the document.
4. Emergency Contact Information:
This contact information is kept on file in the case of an emergency.
5. Employee Confidential Data:
This biographical information is kept on file and is strictly  confidential.
6. Affirmative Action Census: 
FIT is firmly committed to the principles of affirmative action and equal opportunity.  This form is for census purposes, completion is voluntary.
7. W-4:
This is the withholding allowance certificate.  All employees are required to complete this form.
8. Employee's Withholding Allowance Certificate IT-2104:                                This form identifies how much New York State (and New York City and Yonkers) tax to withhold from your check. All employees are required to complete this form in addition to the W-4 form.
8A. Certificate of Exemption from Withholding IT-2104-E                   Employees who are claiming exemption from withholdings on the W-4 form are required to complete this form.
9. Payroll Allocation:
This tells us if you would like your paycheck mailed to you or directly deposited into your bank account.
10. I-9, Employment Eligibility Verification:
All new employees are required to complete this form.

11. Social Security Card:

In order for our Payroll Department to process your paycheck in compliance with Federal regulations, you are required to present your original Social Security Card or a verification letter from the Social Security Administration.
