HEALTH INSURANCE PLANS (Enrollment Optional)
Eligibility:
● All part-time faculty members who are teaching are
eligible to enroll in one of the three health insurance
plans described on the following page.
● You may enroll your spouse or domestic partner*, and your
dependent children up to age 23 (children age 19 or over
must be full-time students to be eligible).
● Please note the enrollment periods for each plan.
● You will be billed on a quarterly basis for the health
insurance plan you choose (the payment will not be deducted
from your paycheck).
● Following three (3) years of employment, part-time faculty
members who teach a minimum of six (6) contact hours per
week will be eligible for a partial reimbursement from the
College for health insurance premiums as noted below.
From Collective Bargaining Agreement, section 35.1.1
There shall be a three (3) year waiting period before
reimbursement of health insurance commences and
reimbursement will only be given to those part-time
classroom faculty who teach a minimum of six (6) hours day,
evening and/or weekend, or a combined program.
For those part-time classroom faculty teaching only day
hours or a combined program, there shall be a twenty percent
(20%) rate per each three (3) hours, prorated. For part-time
classroom faculty teaching evening and/or weekend hours
only, there shall be a fifteen percent (15%) reimbursement
rate per each three (3) hours, prorated, but only when
teaching six (6) hours or more.
Additional Reimbursement Information
For enrollees on the HIP HMO or the HIP POS plan, the
reimbursement noted above is based on the basic premium
rates only (administration fee is not reimbursed).
For enrollees on the Aetna HMO plan, the reimbursement rate
noted above will be based on the Aetna rate if the Aetna
rate is equal to or less than the HIP POS rate. If at any
time the Aetna rate is higher than the HIP POS rate, the
reimbursement will be based on the (lower) HIP POS rate.
*For information on Domestic Partner documentation and the
possible tax consequences of enrolling a Domestic Partner on
the medical plan, please contact the Human Resources

HEALTH INSURANCE PLAN OF NEW YORK (HIP)
(The HIP Plans are Administered by the UCE of FIT Welfare
Fund)
Enrollment Periods: You are eligible to enroll in either the
HIP HMO or HIP POS plans described below during your first
sixty (60) days of employment or during the regular open
enrollment periods of September 1st or January 1st of each
year. In addition, a special enrollment period is available
to individuals who lose other group health insurance
coverage provided application is made within 30 days of the
loss of such coverage. For plan information, provider
directories, or an enrollment application, please contact
the UCE Welfare Fund office at (212)217-3370 or stop by the
office which is located on-campus in room B-902.
HIP PRIME HMO (Health Maintenance Organization)
This plan includes an extensive network of health care
facilities and physicians in the five boroughs of New York
City, plus Nassau, Suffolk, Westchester, Rockland and Orange
counties. You must select a HIP participating primary care
physician who will coordinate your care and referrals to HIP
specialists and hospitals. The plan offers a package of
medical, hospital, and prescription benefits. There is no
out-of-network coverage with this plan except for emergency
care.

HIP POS (Point-of-Service Plan)
Available in the five boroughs of New York City, plus
Nassau, Suffolk, Westchester, Rockland and Orange counties.
This plan offers a package of medical, hospital, and
prescription benefits. The plan allows you to either use
out-of-network providers and hospitals or HIP in-network
providers and hospitals. However, there is an out-of-pocket
annual deductible and co-insurance charges when using
out-of-network providers and hospitals.

AETNA HMO Plan
(The Aetna plan is administered by FIT’s Human Resources
Department)
Enrollment Periods: You are eligible to enroll in the Aetna
HMO plan described below during your first thirty (30) days
of employment or during the annual open enrollment held in
the month of November each year for a January 1st effective
date. In addition, a special enrollment period is available
to individuals who lose other group health insurance
coverage provided application is made within 30 days of the
loss of such coverage. For plan information, provider
directories, or an enrollment application, please contact
the Human Resources Benefits area at (212) 217-3670, or stop
by the office at 236 W. 27th Street, 11th Floor. To view

AETNA HMO (Health Maintenance Organization)
This plan includes an extensive network of physicians and
hospitals throughout the tri-state area of New York, New
Jersey, and Connecticut. You must select an Aetna
participating primary care physician who coordinates your
care and referrals to specialists and hospitals. The plan
offers a package of medical, hospital, and prescription
benefits. Under this plan, office visits are covered for a
$10 co-pay. There is no out-of-network coverage with this
plan except for emergency care.

RETIREMENT PLANS (ENROLLMENT OPTIONAL)
Part-time faculty employees may join one or both of the
following retirement plans. Note: A decision not to enroll
as a new faculty members will not disqualify you from
enrolling at a future date. However, you must complete the
Retirement Option form indicating
whether you are electing to join the New York State
Teachers’ Retirement System as a new employee.
NEW YORK STATE TEACHERS’ RETIREMENT SYSTEM (“NYSTRS”) -
(Enrollment Optional)
(Defined Benefit Plan)
FIT is required by New York State legislation to obtain from
all new part-time faculty members, in writing, whether they
wish to join or not join the New York State Teachers’
Retirement System. It is mandatory that you indicate your
decision, sign, and return a NYSTRS Retirement Option form
to Human Resources as soon as possible.
If you join the NYSTRS, your salary will be reduced by 3%
each pay period as your employee contribution for the first
ten (10) years of membership; no further employee
contributions are required thereafter. FIT as your employer,
also makes a contribution to the plan on your behalf. A
booklet from the NYSTRS describing membership, benefits, and
plan guidelines is available in Human Resources. You may
also obtain additional information from the NYSTRS web site
www.nystrs.org.

SUPPLEMENTAL RETIREMENT ANNUITY PLAN (Enrollment Optional)
A Supplemental Retirement Annuity (SRA) 403(b) plan is a
tax-deferred annuity for employees who want to set aside a
portion of their salary for long term tax-deferred
retirement accumulations. (The College does not make any
contribution to this plan.) An employee may elect to reduce
his/her salary by a given percentage, as long as this amount
does not exceed the dollar amount permitted by the Internal
Revenue Code’s maximum exclusion allowance. The IRS maximum
exclusion allowance limits for calendar year 2010 are
$16,500 for employees under age fifty and $22,000 for
employees age 50 and over. Employees may make up to four
salary reductions changes per calendar year.
Participants in the SRA plan may direct salary reduction
monies to one of the following five retirement carriers or
divide SRA monies among any number of the four carriers
shown below.
403 (b) Carriers: ING, Met Life, TIAA-CREF,
VALIC
Each carrier offers a variety of investment options.
Booklets for each of the carriers are available in Human
Resources and may be obtained by calling the Benefits area
at extension 73670. Representatives from each carrier are
available on campus throughout the spring and fall semesters
to meet individually with employees to answer questions on
investment options and to assist in retirement planning. A
schedule which lists the days each representative will be on
campus is circulated each semester by the Benefits area.

TRANSITCHEK PROGRAM
All employees of the College are eligible to participate in
the TransitChek program. The program allows employees to
elect to have up to a maximum of $230 per month ($2,760
annually) deducted from payroll checks on a pre-tax basis to
use for mass-transit transportation expenses. This results
in a tax-savings to the employee. TransitChek enrollees may
choose to receive benefits in the form of the Premium
TransitChek Metro Card, railroad Mail-and-Ride credits, or
Voucher checks which can be used with other participating
bus/train/van/ferry providers. The enclosed brochure
describes the program in more detail and also provides a
complete listing of the participating transit providers. You
may enroll by calling Transit Center directly at 1-800
557-0553 or by enrolling over their secure web site at
www.transitchekdirect.com. To complete your enrollment, you will
need to provide FIT’s company code which is FTN28, and also
the FIT employee ID number you were issued. Please note that
there is a minimum waiting period of six weeks between the
time of enrollment and when the first benefit is received.

TUITION EXEMPTION FOR COURSES TAKEN AT FIT
Part-time faculty teaching a minimum of six (6) contact
hours per week, their spouse or domestic partner, and
dependent children, shall be exempt from paying tuition and
registration fees for all courses offered at FIT, subject to
IRS guidelines. Tuition Exemption forms are available in
Human Resources and should be completed and approved prior
to registering for classes. Your department chairperson will
need to the sign the form verifying the number of contact
hours you are teaching for the semester. In additional, the
form will need to be approved in Human Resources.

TUITION ASSISTANCE FOR COURSES TAKEN AT OTHER SUNY COLLEGES
This benefit applies to FIT employees only and is not
available to family members. Each year FIT receives an
allotment from SUNY to provide tuition assistance to
employees taking courses at other SUNY campuses. Employees
must pay full tuition costs upon registration and then
submit proof of registration and payment to the Human
Resources Department by April 1st each year. The amount of
assistance each employee receives will depend on the total
number of employees who apply. For further information and
to obtain SUNY tuition assistance forms, please call the
Human Resources Benefits area at extension 73670.

EMPLOYEE
ASSISTANCE PROGRAM (EAP)
The FIT/UCE Employee Assistance Program is a jointly
sponsored labor/management program. It is a free service
designed to assist employees who may be experiencing
personal problems which can affect their ability to function
on the job. The program is also available to the family
members or significant others of FIT employees. The FIT/UCE
EAP program upholds the strictest professional and legal
standards of confidentiality. Please see the enclosed
brochure for more details on the program. The EAP

WELLNESS PROGRAM
The Employee Wellness Program offers a variety of programs
each semester including free lunchtime exercise classes,
Weight Watchers groups, special lunchtime seminars, and
health screenings. Fliers announcing the events are
circulated on campus.

WORKERS’ COMPENSATION
FIT employees are covered under the New York State Workers’
Compensation law. This program provides payment for all
necessary and reasonable medical care expenses related to
your on-the-job injury or occupational disease. This program
may also provide reimbursement for lost wages at rates set
by the New York State Department of Labor. Employees should
immediately report any accident or illness to FIT Security
(Ext. 77777) and/or the FIT Health Services (Ext. 77625) and
seek first aid or other necessary medical attention. The
Human Resources Department Benefits area administers the
Workers’ Compensation program for FIT employees. Our
Workers’ Compensation carrier is PMA Management Corp., 5789
Widewaters Parkway, Dewitt, NY 13214. They can be reached at
