Medical Records
Medical records can be released with written authorization from the patient. If patient is less than 18 years of age, a written authorization from a parent or legal guardian is required.
Click here to download a Medical Release Authorization form. The form must be filled out in person at the Health Services. If that is not possible, the patient may fax a notarized copy of the completed Medical Release Authorization form to 212.217.4191.
