Health Insurance

There is a mandatory hard waiver health insurance policy.  All full-time students are automatically billed for this health insurance.  For questions and details of the plan, go to www.gallagherkoster.com or call Gallagher Koster at 1.800.397.4939.
 

Students can be excused from the mandatory insurance if they have equal or better health coverage from another company.  In order to waive the mandatory insurance, students must submit an online waiver form by the waiver deadline date.

Instructions to waive the student health insurance:

• Go to www.gallagherkoster.com.
• Click STUDENT ACCESS.
• Select "Fashion Institute of Technology" from the drop-down menu, then click CONTINUE.
• Click LOGIN on the left menu.  First time users are required to create an account.  To this you will need to use your first and last name, student ID, date of birth and email address.  If you are a returning student, you will be able to use your existing User Account information.
• Once logged in, click STUDENT WAIVE/ENROLL FORMS on the left menu.
• Select the 2008-2009 FIT ANNUAL WAIVER FORM from the drop-down menu.

Immediately upon submitting the waiver form, you will receive a confirmation number.  You will subsequently receive an email confirmation at the email address you provided.  Retain this confirmation number for your records, as it is your only documentation that your form was successfully submitted.

Prescriptions

For coverage, present your ID card at the pharmacy when dropping off your prescription.  If you haven't received your ID card, you may pay for the prescription, then complete the Medco Pharmacy Reimbursement Form for reimbursement.

Need insurance after graduation?

You may be eligible to enroll for one additional period of coverage through Gallagher Koster.  You have to be enrolled in the student health insurance plan for at least 6 consecutive months to be eligible to continue your coverage after it terminates.  Enrollment must be made at least 15 days prior to termination.  The Continuation Plan does not duplicate the coverage of your Student Health Insurance Plan and the premium is not the same.  You are responsible for timely payment of the premium.  Call Gallagher Koster at 1.800.397.4939 for more information and enrollment details.
