WHAT IS THE PARENTAL CONSENT FORM?

 All students need to submit and Information and Consent Form (Parental Consent Form) signed by your parent or guardian before they can participate in our program.  This form provides FIT with important information in case of a medical emergency. Students will not be permitted to attend class if this form is not in our records.
NO FORM = NO CLASSES

You must submit this form during Orientation OR as you enter the building on the first day of class.
