Change Of Address:

Please submit the Change of Address form as soon as you have relocated in order to prevent misdirected mail. 


Change Of Major Request:

Use this form only if you have already applied and now wish to change from the major originally indicated on your application to a new major.

 


Residency:

Changes in Residency Status can only be completed after a student has been admitted. If you have received an admissions letter and believe you are eligible for the instate tuition rate, please follow the instructions on the following link: Bursar's Office: Residency     


Request To Withdraw My Application:

If you have applied to FIT and no longer wish to attend, please complete this form to remove your application from active consideration.  Please note that if you would like to be considered for a future semester, you must submit an entirely new application.
