By submitting this form, you will no longer be considered for the major initially chosen on your application.  Applicants are only eligible to submit this request if an admissions decision has not been rendered.  In addition, Art and Design majors will be required to submit a portfolio for review.  If a portfolio is required, the Office of Admissions will contact you with instructions.  If this form is submitted after the application deadline (January 1 for fall and October 1 for spring), priority consideration cannot be guaranteed.
